Sure Start Early Years Outreach Service

Early Years Outreach Workers provide support, intervention and guidance to families in the community to access both universal and targeted services using an Early Help Assessment Form. Where additional needs have been identified, requests for support are made from a wide range of agencies and by parents for Early Years Outreach Workers to;

 

  • Provide information about groups and activities for children under 5 years in the Sure Start neighbourhood
  • Provide evidence based interventions, i.e. WellComm, Parent Communication Language groups, Child and Parent Service
  • Support children to access 2 yearr and 3 year education funding entitlement
  • Support parent to find childcare, school places
  • Support to parents who are new to the area to signpost to relevant services, i.e. registering with GP, Dentist, Library, Sure Start Children’s Centre activities
  • Contact parents to advise of appointment dates and times to avoid DNA’s and accompany to appointments if required
  • Advise and provide information about adult learning and employment opportunities. (Support links with job centre, work clubs and work coach)
  • Provide information about volunteering opportunities and recruit parents to become volunteers, access to training and employment
  • Working with parents and children in the home or in groups
  • Support to manage children’s behaviour, recruit and deliver ‘Parent Survival’ courses including home visiting for course attendees
  • Information, advice or signposting to Community Nursery Nurse for support on weaning, immunisations, breastfeeding, toilet training, and healthy eating
  • Child safety information, safety in the home assessments and referrals for free home safety equipment (subject to eligibility).
  • Support for families in hardship to get essential household items including white goods, furniture / clothing / baby equipment (subject to eligibility)
  • Support to parents who are isolated in the community, accompany to groups
  • Referrals to other agencies for additional support such as
    • adult education,
    • housing,
    • counselling,
    • benefit advice,
    • Women’s Aid
    • Homeless Families
    • Citizens Advice Bureau e.g. Debt Management
  • Support and assistance to complete and fill in forms including on-line applications for housing, school places etc.
